NHS Type 2 Diabetes Path To Remission Programme
For people living with type 2 diabetes
The 12-month programme is dedicated to guiding and supporting you throughout your weight loss journey using a total diet replacement plan, followed by supported food reintroduction and weight loss maintenance. In addition, the programme aims to lower your blood glucose levels, reduce your dependence on certain medications, and potentially put your type 2 diabetes into remission.

The Counterweight Plus programme was used in the DiRECT clinical trial, the world’s largest ever study of type 2 diabetes remission funded by Diabetes UK. The programme was so successful that it made waves in the medical world and changed the understanding of treatment and management of type 2 diabetes around the world.
Am I eligible for the NHS Type 2 Diabetes Path to Remission Programme?
- People aged between 18 and 65 years old
- Confirmed diagnosis of type 2 diabetes within the last 6 years
- Those who are motivated to change their lifestyle and can commit to the 12-month programme
- Have a BMI of 27 kg/m2 or over (White ethnic groups) and 25 kg/m2 or over (Black, Asian and other ethnic groups)
- Currently living in North West London, Shropshire, Telford, Wrekin, Suffolk, North East Essex, Cornwall or the Isles of Scilly

How does the programme work?
This is a 12-month programme led by a team of experts (dietitians, nutritionists, health coaches, psychologists and a medical team) and covers three programme stages
- You will be offered low-calorie, total diet replacement products (soups, shakes and porridge) totalling 800 to 900 kilocalories per day for up to 12 weeks. During this time you will be expected to replace all your every day meals with these products
- Alongside this, you will receive support and monitoring for 12 months, including help to reintroduce food after the initial 12-week period
- You can choose how the programme is delivered to best suit your needs. This will be one-to-one support, delivered either:
- In-person; or
- Remote via phone calls / video calls - This support will provide you with all the tools you need throughout every phase of the programme. Your GP practice will be kept informed of your progress and if any medication changes might be needed
